At the heart of every effective survey is a clear set of questions. Unclear questions give unclear answers. Specifically, when developing a community health assessment survey, you need to know what questions to ask in order to elicit the pertinent information. It is also important that you know how questions should be phrased in order to be understood by the receiver. A survey protocol contains a copy of the survey instrument(s) to be used and describes details of the methods and procedures to be used for data collection and management.

Review this week’s Resources to gather information about developing an effective survey protocol. You then apply this information into creating a survey protocol for your Scholar-Practitioner Project. When developing your survey protocol questions, be sure to consider whether you would use a quantitative, qualitative, or mixed method.

The Assignment (4–5 pages):

Submit by Day 7 a 4- to 5-page protocol for a survey to gather information about the problem in your Scholar-Practitioner Project. The protocol should include the following:

A brief set of objectives for the survey

Describe the population you would sample and explain why.

Describe your data collection method you might use and explain why.

Describe your data analysis method and explain why.

A 1-page (10–20 questions) questionnaire for the survey.

Use the Resources and current literature to support your response.

(Note: The students will not be conducting the survey, only preparing the protocol.)

Support your Scholar-Practitioner Project Assignment with specific references to all resources used in its preparation. You are to provide a reference list for all resources, including those in the Learning Resources for this course.
